OF NORTH CENTRAL FLORIDA
Rural Health Partnership is a community-based, nonprofit serving nine counties in North Central Florida. WellFlorida Council is our organizational partner and the health planning council for the region. WellFlorida provides staff support and services such as health planning and research, community organizing, health promotion, disease prevention and care coordination.
Rural Health Partnership’s mission, as one of nine independent rural health networks in Florida (per State Statute 381.0406), is to provide for the efficient and effective delivery of health services in the organization’s service area through the integration of public and private resources and the coordination of healthcare providers.
Our service area includes Bradford, Columbia, Dixie, Gilchrist, Hamilton, Levy, Union, Suwannee counties and those census tracts of Alachua County with a population density of 100 persons or less per square mile.
